    An Interactive WebGIS Framework for Coastal Erosion Risk Management

    The Italian coastline stretches over about 8350 km, with 3600 km of beaches, representing a significant resource for the country. Natural processes and anthropic interventions keep threatening its morphology, moulding its shape and triggering soil erosion phenomena. Thus, many scholars have been focusing their work on investigating and monitoring shoreline instability. Outcomes of such activities can be largely widespread and shared with expert and non-expert users through Web mapping. This paper describes the performances of a WebGIS prototype designed to disseminate the results of the Italian project Innovative Strategies for the Monitoring and Analysis of Erosion Risk, known as the STIMARE project. While aiming to include the entire national coastline, three study areas along the regional coasts of Puglia and Emilia Romagna have already been implemented as pilot cases. This WebGIS was generated using Free and Open-Source Software for Geographic information systems (FOSS4G). The platform was designed by combining Apache http server, Geoserver, as open-source server and PostgreSQL (with PostGIS extension) as database. Pure javascript libraries OpenLayers and Cesium were implemented to obtain a hybrid 2D and 3D visualization. A user-friendly interactive interface was programmed to help users visualize and download geospatial data in several formats (pdf, kml and shp), in accordance with the European INSPIRE directives, satisfying both multi-temporal and multi-scale perspectives. © 2021 by the authors.     Predicting land use change on a broad area: Dyna-CLUE model application to the Litorale Domizio-Agro Aversano (Campania, South Italy)

    The long-standing awareness of the environmental impact of land-use change (LUC) has led scientific community to develop tools able to predict their amount and to evaluate their effect on environment, with the aim supporting policy makers in their planning activities. This paper proposes an implementation of the Dyna-CLUE (Dynamic Conversion of Land Use and its Effects) model applied to the Litorale Domizio-Agro Aversano, an area of Campania region, which needs interventions for environmental remediation. Future land use changes were simulated in two different scenarios developed under alternative strategies of land management: scenario 1 is a simple projection of the recent LUC trend, while scenario 2 hypothesises the introduction of no-food crops, such as poplar (Populus nigra L.) and giant reed (Arundo donax L.), in addition to a less impactful urban sprawl, which is one of the main issues in the study area. The overall duration of simulations was 13 years, subdivided into yearly time steps. CORINE land cover map of 2006 was used as baseline for land use change detection in the study area. Competition between different land use types is taken into account by setting the conversion elasticity, a parameter ranging from 0 to 1, according to their capital investment level. Location suitability for each land use type is based on logit model. Since no actual land use already exists for the alternative crops investigated in scenario 2, a suitability map realised through a spatial multicriteria decision analysis was used as a proxy for its land use pattern. The comparison of the land use in 2012 and scenario 1, evaluated through the application of Kappa statistics, showed a general tendency to expansion of built-up areas, with an increase of about 2400 ha (1.5% of the total surface), at the expense of agricultural land and those covered by natural vegetation. The comparison of the land use in 2012 and scenario 2 showed a less significant spread of built-up areas, affecting approximately 750 ha (0.5% of the total surface). Moreover, the introduction of alternative crops on about 10,000 ha, that is 6.8% of the total surface, would result in a significant decrease of arable land and a lower decrease of permanent crops, respectively equal to 6800 ha and 2900 ha. This work highlighted the importance and the potential of predicting land-use change models as valid tools supporting decisions, especially in those regions needing interventions aimed to environmental remediation, as in the case study examined in this paper

    Standardized postnatal management of infants with congenital diaphragmatic hernia in Europe: The CDH EURO Consortium Consensus - 2015 Update

    In 2010, the congenital diaphragmatic hernia (CDH) EURO Consortium published a standardized neonatal treatment protocol. Five years later, the number of participating centers has been raised from 13 to 22. In this article the relevant literature is updated, and consensus has been reached between the members of the CDH EURO Consortium. Key updated recommendations are: (1) planned delivery after a gestational age of 39 weeks in a high-volume tertiary center; (2) neuromuscular blocking agents to be avoided during initial treatment in the delivery room; (3) adapt treatment to reach a preductal saturation of between 80 and 95% and postductal saturation >70%; (4) target PaCO2 to be between 50 and 70 mm Hg; (5) conventional mechanical ventilation to be the optimal initial ventilation strategy, and (6) intravenous sildenafil to be considered in CDH patients with severe pulmonary hypertension. This article represents the current opinion of all consortium members in Europe for the optimal neonatal treatment of CDH

    CERT1 mutations perturb human development by disrupting sphingolipid homeostasis

    Neural differentiation, synaptic transmission, and action potential propagation depend on membrane sphingolipids, whose metabolism is tightly regulated. Mutations in the ceramide transporter CERT (CERT1), which is involved in sphingolipid biosynthesis, are associated with intellectual disability, but the pathogenic mechanism remains obscure. Here, we characterize 31 individuals with de novo missense variants in CERT1. Several variants fall into a previously uncharacterized dimeric helical domain that enables CERT homeostatic inactivation, without which sphingolipid production goes unchecked. The clinical severity reflects the degree to which CERT autoregulation is disrupted, and inhibiting CERT pharmacologically corrects morphological and motor abnormalities in a Drosophila model of the disease, which we call ceramide transporter (CerTra) syndrome. These findings uncover a central role for CERT autoregulation in the control of sphingolipid biosynthetic flux, provide unexpected insight into the structural organization of CERT, and suggest a possible therapeutic approach for patients with CerTra syndrome.This work was supported by the National Institute of Neurological Disorders and Stroke (NINDS), NIH (R01NS109858, to VAG); the Paul A.     How future surgery will benefit from SARS-COV-2-related measures: a SPIGC survey conveying the perspective of Italian surgeons

    COVID-19 negatively affected surgical activity, but the potential benefits resulting from adopted measures remain unclear. The aim of this study was to evaluate the change in surgical activity and potential benefit from COVID-19 measures in perspective of Italian surgeons on behalf of SPIGC. A nationwide online survey on surgical practice before, during, and after COVID-19 pandemic was conducted in March-April 2022 (NCT:05323851). Effects of COVID-19 hospital-related measures on surgical patients' management and personal professional development across surgical specialties were explored. Data on demographics, pre-operative/peri-operative/post-operative management, and professional development were collected. Outcomes were matched with the corresponding volume. Four hundred and seventy-three respondents were included in final analysis across 14 surgical specialties. Since SARS-CoV-2 pandemic, application of telematic consultations (4.1% vs. 21.6%; p < 0.0001) and diagnostic evaluations (16.4% vs. 42.2%; p < 0.0001) increased. Elective surgical activities significantly reduced and surgeons opted more frequently for conservative management with a possible indication for elective (26.3% vs. 35.7%; p < 0.0001) or urgent (20.4% vs. 38.5%; p < 0.0001) surgery. All new COVID-related measures are perceived to be maintained in the future. Surgeons' personal education online increased from 12.6% (pre-COVID) to 86.6% (post-COVID; p < 0.0001). Online educational activities are considered a beneficial effect from COVID pandemic (56.4%). COVID-19 had a great impact on surgical specialties, with significant reduction of operation volume. However, some forced changes turned out to be benefits. Isolation measures pushed the use of telemedicine and telemetric devices for outpatient practice and favored communication for educational purposes and surgeon-patient/family communication. From the Italian surgeons' perspective, COVID-related measures will continue to influence future surgical clinical practice

    Colorectal Cancer Stage at Diagnosis Before vs During the COVID-19 Pandemic in Italy

    IMPORTANCE Delays in screening programs and the reluctance of patients to seek medical attention because of the outbreak of SARS-CoV-2 could be associated with the risk of more advanced colorectal cancers at diagnosis. OBJECTIVE To evaluate whether the SARS-CoV-2 pandemic was associated with more advanced oncologic stage and change in clinical presentation for patients with colorectal cancer. DESIGN, SETTING, AND PARTICIPANTS This retrospective, multicenter cohort study included all 17 938 adult patients who underwent surgery for colorectal cancer from March 1, 2020, to December 31, 2021 (pandemic period), and from January 1, 2018, to February 29, 2020 (prepandemic period), in 81 participating centers in Italy, including tertiary centers and community hospitals. Follow-up was 30 days from surgery. EXPOSURES Any type of surgical procedure for colorectal cancer, including explorative surgery, palliative procedures, and atypical or segmental resections. MAIN OUTCOMES AND MEASURES The primary outcome was advanced stage of colorectal cancer at diagnosis. Secondary outcomes were distant metastasis, T4 stage, aggressive biology (defined as cancer with at least 1 of the following characteristics: signet ring cells, mucinous tumor, budding, lymphovascular invasion, perineural invasion, and lymphangitis), stenotic lesion, emergency surgery, and palliative surgery. The independent association between the pandemic period and the outcomes was assessed using multivariate random-effects logistic regression, with hospital as the cluster variable. RESULTS A total of 17 938 patients (10 007 men [55.8%]; mean [SD] age, 70.6 [12.2] years) underwent surgery for colorectal cancer: 7796 (43.5%) during the pandemic period and 10 142 (56.5%) during the prepandemic period. Logistic regression indicated that the pandemic period was significantly associated with an increased rate of advanced-stage colorectal cancer (odds ratio [OR], 1.07; 95%CI, 1.01-1.13; P = .03), aggressive biology (OR, 1.32; 95%CI, 1.15-1.53; P < .001), and stenotic lesions (OR, 1.15; 95%CI, 1.01-1.31; P = .03). CONCLUSIONS AND RELEVANCE This cohort study suggests a significant association between the SARS-CoV-2 pandemic and the risk of a more advanced oncologic stage at diagnosis among patients undergoing surgery for colorectal cancer and might indicate a potential reduction of survival for these patients

    Accuracy Assessment of Cultural Heritage Models Extracting 3D Point Cloud Geometric Features with RPAS SfM-MVS and TLS Techniques

    A proper classification of 3D point clouds allows fully exploiting data potentiality in assessing and preserving cultural heritage. Point cloud classification workflow is commonly based on the selection and extraction of respective geometric features. Although several research activities have investigated the impact of geometric features on classification outcomes accuracy, only a few works focused on their accuracy and reliability. This paper investigates the accuracy of 3D point cloud geometric features through a statistical analysis based on their corresponding eigenvalues and covariance with the aim of exploiting their effectiveness for cultural heritage classification. The proposed approach was separately applied on two high-quality 3D point clouds of the All Saints’ Monastery of Cuti (Bari, Southern Italy), generated using two competing survey techniques: Remotely Piloted Aircraft System (RPAS) Structure from Motion (SfM) and Multi View Stereo (MVS) techniques and Terrestrial Laser Scanner (TLS). Point cloud compatibility was guaranteed through re-alignment and co-registration of data. The geometric features accuracy obtained by adopting the RPAS digital photogrammetric and TLS models was consequently analyzed and presented. Lastly, a discussion on convergences and divergences of these results is also provided

    Estimating Plant Traits of Grasslands from UAV-Acquired Hyperspectral Images: A Comparison of Statistical Approaches

    Grassland ecosystems cover around 40% of the entire Earth’s surface. Therefore, it is necessary to guarantee good grassland management at field scale in order to improve its conservation and to achieve optimal growth. This study identified the most appropriate statistical strategy, between partial least squares regression (PLSR) and narrow vegetation indices, for estimating the structural and biochemical grassland traits from UAV-acquired hyperspectral images. Moreover, the influence of fertilizers on plant traits for grasslands was analyzed. Hyperspectral data were collected from an experimental field at the farm Haus Riswick, near Kleve in Germany, for two different flight campaigns in May and October. The collected image blocks were geometrically and radiometrically corrected for surface reflectance. Spectral signatures extracted for the plots were adopted to derive grassland traits by computing PLSR and the following narrow vegetation indices: the MERIS Terrestrial Chlorophyll Index (MTCI), the ratio of the Modified Chlorophyll Absorption in Reflectance and Optimized Soil-Adjusted Vegetation Index (MCARI/OSAVI) modified by Wu, the Red-edge Chlorophyll Index (CIred-edge), and the Normalized Difference Red Edge (NDRE). PLSR showed promising results for estimating grassland structural traits and gave less satisfying outcomes for the selected chemical traits (crude ash, crude fiber, crude protein, Na, K, metabolic energy). Established relations are not influenced by the type and the amount of fertilization, while they are affected by the grassland health status. PLSR is found to be the best strategy, among the approaches analyzed in this paper, for exploring structural and biochemical features of grasslands.
